Why the safest answer is not always the keyed one
Nursing items are written so that more than one option is defensible and only one is the priority. Being told which letter was correct leaves you no better placed on the next item. The tutor asks what the stem requires first, then works through why each plausible option is not it, which is the reasoning being measured.
