Organisational Change and Learning Systems

Organisational transformation changes structures, roles or operating logic rather than digitising an existing step. This chapter assesses readiness through shared purpose, capability, capacity, leadership, infrastructure, trust and fit. It then designs a learning health system loop from data to knowledge, knowledge to performance and performance back to data.

Sustainment depends on governance, feedback and the capacity to adapt safely after the project team leaves. Test transformational claims by describing future work. State which roles, routines, relationships and information flows change, what stops, what begins and what people must learn. Readiness is not positive attitude.

A team can support the purpose while lacking time, confidence, role clarity, infrastructure or response capacity. Diagnose these conditions separately and reassess them because participation, pilot experience and organisational change can alter readiness over time. A learning health system closes a governed feedback loop.

Data must be meaningful and timely for a question, analysis must preserve uncertainty, knowledge must reach a decision point and changed practice must generate new evidence. The core values in the course materials become operating requirements: person focus, privacy, inclusion, transparency, accessibility, adaptability, governance, participatory leadership, scientific integrity and value. A dashboard alone cannot deliver them.

Sustainment requires operational, clinical safety and data ownership plus a review rhythm. Use a compact set of reach, workflow, outcome, experience, equity and safety signals, giving each an owner and response rule. Record adaptations and distinguish routine adjustment from a change requiring formal reassessment. Close the plan with conditions for scale, targeted modification, further evidence or pause.

This shows that the service retains the capacity to notice, explain and respond when context changes.

What distinguishes transformation from improvement?

Improvement adjusts an existing process. Transformation changes the structure, roles, relationships or operating logic of a system. A new interface is not transformational unless it changes how work and decisions are organised.

Is readiness the same as positive attitude?

No. Readiness includes shared purpose, confidence, skills, time, infrastructure, role clarity, leadership, trust and fit with priorities. A team can support the goal while lacking the capacity required for safe launch.

When does a feedback loop become a learning system?

The loop must connect meaningful data to credible interpretation, a decision forum, an accountable change in practice and new evidence. Data collection or a dashboard without response capacity leaves the loop open.

How should a project plan for sustainment?

Name operational, safety and data owners; define a review rhythm; keep a small action-oriented measure set; record adaptations; and state the conditions for scale, further evidence, modification or pause.

Write a future-work description that names changed roles, routines and information flows. Assess readiness with evidence rather than enthusiasm. Draw the learning cycle for your project and label the data source, interpretation method, decision forum, action owner and next measurement point. Build a sustainment table with review frequency, response rule and adaptation authority.

Finish by explaining which signal would justify scale and which would require pause. Separate conditions required before launch from capabilities that can grow during supported use. Test the cycle for an open-loop failure: data collected without interpretation, knowledge produced without a decision forum, or a decision made without new measurement. Assign operational, safety and data owners and distinguish their authority.

Rehearse one adaptation that preserves core function and one modification serious enough to require formal reassessment. Close with a short governance calendar for early launch and routine operation. Include a handover test: a new owner should be able to explain the current assumptions, unresolved risks and next review without relying on the original project team.

Draft the first meeting agenda after handover and make every dashboard item lead to a named question, decision or action. Remove metrics that nobody can interpret or influence, and record where qualitative evidence is needed to explain an unexpected signal.
