Attention and Cognitive Load in Clinical Teaching

One step earlier than retrieval

The third session begins before anything can be recalled. Recall strengthens whatever got in, getting in at all requires attention, and attention is picky, finite, and steered by what the learner is trying to do, by what stands out, and by what they already know.

The session states the consequence bluntly: exposure is not the same thing as attention, and what is not selected receives little opportunity to be encoded at all.

Three constraints are named separately because they call for different responses. Selection is the problem of identifying what matters among competing signals, and it is worst when everything on a page looks equally important.

Sustained focus fluctuates with fatigue, stress, interruption and task length, which makes it a scheduling problem rather than a content problem.

Control is the capacity to ignore noise, and it is unevenly distributed: prior knowledge lets an expert filter a busy scene that a novice cannot, so the same environment imposes very different demands on two people standing in it.

Four moves that direct attention without removing the thinking

Orient by stating the clinical question or decision first, so the learner knows what the information is for before receiving it.

Organise by grouping what follows into a meaningful structure rather than a list. Stop at real boundaries so there is room to think, to guess ahead, or to sum up. Check by asking learners to retrieve the key point, which is both an attention device and retrieval practice arriving inside the session.

Underneath all four sits a narrow question worth asking literally: what should learners be looking at at this exact moment, and what should they be doing with it?

Three loads, three different responses

Cognitive Load Theory connects the memory limits of the previous chapter to design.

Intrinsic load reflects the interacting elements the task genuinely contains, and it is managed by sequencing and scaffolding rather than removed. Extraneous load is whatever the arrangement adds on top and could have left out: attention divided between two places, the same thing said twice, weak ordering, decorative detail.

Germane load is the part of the effort that goes into explaining, into assembling a structure, and into reasoning through the case, and the other two crowd it out. The discipline is to reduce the avoidable load and support the essential thinking, because the ceiling does not move.

The reason this matters is that the same symptom, a learner who cannot follow, arises from opposite causes.

High intrinsic load calls for sequencing, since the complexity is the content. High extraneous load calls for removal, since nothing is lost. Treating the first as the second produces a course simplified into uselessness; treating the second as the first produces an educator convinced the topic is inherently hard.

Clinical communication as the visible case

Handover is where load becomes observable.

A complete, chronological handover can present a large number of elements at equal emphasis, in an order set by the notes rather than by the decisions the receiver must make, with no pause at which anything consolidates and no point at which the receiver says back what matters. Working memory holds a few items at a time, so an action arriving mid-stream is displaced by what follows.

Reordering the same content around problem, change, risk and action, and then inviting confirmation, reduces the number of elements held simultaneously and places the action where it is protected. Nothing is added or removed, which is what makes it a design problem rather than a knowledge problem.

The intrinsic load is whatever the clinical principle genuinely requires, the interacting elements a learner must hold together, and it cannot be removed without removing the content. The extraneous load is created here: two visual sources competing for the same attention, an explanation that does not correspond to the displayed slide, and note-taking imposed at the moment when processing should happen. Germane load is squeezed out, because capacity is consumed reconciling two displays. The redesign removes the competing slide, signals the single relevant cue, segments the explanation at a natural boundary, and asks for a prediction before the answer is given, which converts the freed capacity into germane effort rather than leaving it idle.

Why is a complete handover sometimes a failed handover?

Completeness is about content and the failure is about structure. Presenting many facts at equal emphasis, ordered by when they happened, exceeds what the receiver can hold, so an action mentioned in the middle is displaced by what follows. Reordering around the decisions the receiver must take fixes it without changing a single fact.

How do you tell intrinsic load from extraneous load in practice?

Ask whether the difficulty tracks the clinical content or the presentation. If experienced people in the same room also lose the thread, the presentation is the cause and the effort can be removed with nothing lost. If only novices struggle while the material is cleanly presented, the complexity is the content and the answer is sequencing.

Does reducing load conflict with making learning effortful?

No, because they target different effort. The demand worth keeping is the effort of generating an answer or reasoning through a case. The demand worth removing is the effort of reconciling a badly arranged display. Cutting the second creates the capacity the first needs.

Why does prior knowledge change how demanding a scene is?

Organised knowledge in long-term memory lets several elements be handled as one, so an expert holds a clinical picture where a novice holds a list of separate findings. The room has not changed; what has changed is how many units the learner must track at once.

What does checking add beyond keeping attention?

Asking learners to retrieve the key point converts a passive moment into a recall attempt, so it earns the benefit of the previous chapter inside the current session. It also returns information about whether the signalling worked, which a nod does not.
