Why a keyed answer teaches you nothing about the next item
NBCOT items are built so that several options are clinically reasonable and only one is keyed. Being told which letter was right leaves you no better placed on the next item. The tutor asks what the stem was asking you to do first, then works through why each plausible option fails, which is the reasoning the exam is actually measuring.
Personal here means it remembers which domain keeps failing, not just how many items you missed. If your intervention planning is strong but precaution screening collapses when the stem is long, those are tracked separately, and practice days later returns on the second one.

Yes. The domain coverage spans both, and practice can be weighted to the credential you are sitting rather than spread across both.
Why do I get items wrong that I would answer correctly on placement?
Because the exam is testing one specific domain and one specific qualifier, and several options remain defensible in practice. The tutor teaches you to identify the domain and the qualifier first, which is the gap between clinical competence and exam performance.
Can it explain the rationale for every option?
