From Evidence to a Bounded Case Formulation

Fix scale, actor and purpose

A formulation is not a document list or disguised diagnosis. It selects patterns relevant to the decision and explains possible connections. Separate history, current triggers, maintaining cycles and protective resources. Mark each link's evidence level. Alternative hypotheses stop every observation being fitted to the first story.

End with next questions and proportionate supports while uncertainty remains.

The chapter objective is to link presenting patterns, vulnerabilities, triggers, maintaining processes, protections and unanswered questions. Begin by defining case formulation at the scale used in the question.
