SOAP Notes from Encounter to Plan

SOAP preserves the reasoning route

Subjective contains the patient's report, Objective contains observable or verified data, Assessment interprets those sources, and Plan states action, goals, monitoring and follow-up. A strong note lets another clinician trace every action backward to its reason.

Concision removes irrelevant detail without removing the evidence or uncertainty that makes the plan defensible.

Audit in both directions

Read forward for unsupported leaps and backward for plan items with no evidential parent.

Protect information status

The patient's account remains subjective even when persuasive. A measured or verified finding remains objective.

The pharmacist's interpretation belongs in Assessment. Keeping these statuses separate lets a reader distinguish what happened, what was observed and what was concluded.

Earn the plan

A problem label alone does not explain action. State problem status, supporting evidence, relevant uncertainty and immediate risk. The assessment can be concise when relationships are explicit.

It becomes long and weak when facts are repeated without showing why they matter.

Make action reproducible

Record the action, goal, monitoring measure, review interval, responsible person and trigger for earlier contact. Generic verbs such as counselled or follow up describe activity without describing intended change.

Another clinician should know what to check.

Edit after tracing

Verify the forward route from S and O through A to P, then trace every P item backward. Remove repeated detail only after the clinical logic works. Concision should expose reasoning, not hide source, uncertainty or monitoring.

Audit the whole note

Choose one assessed problem and follow it across all four sections.

If the route breaks, repair the earliest missing link. Adding detail at the end cannot compensate for evidence or interpretation that never appeared upstream.

Documenting repeated missed doses

Q [6 marks]. Write the reasoning core after variable shifts disrupt evening use. The step weighting shown here is an AskSia study allocation and is not a University marking scheme.

  • 2Place report and verified data correctly.
  • 2State the barrier and uncertainty in Assessment.
  • 2Write an auditable Plan.
Put the missed-use pattern and reason in Subjective, the verified regimen in Objective, and the practical barrier plus unresolved clinical consequence in Assessment. The Plan should name support, review responsibility, the response to monitor and the condition for earlier contact.
Sia tip — If a plan item appears from nowhere, repair Assessment before expanding Plan.
Glossary

Key terms

SOAP Note
A clinical note structured as Subjective, Objective, Assessment and Plan.
Problem Status
A concise judgement about whether a problem is controlled, changing or unresolved.
Traceability
The visible link from evidence through assessment to each plan item.
FAQ

SOAP Notes from Encounter to Plan FAQ

Where should the history of the present problem go?

Follow the course's local documentation convention and keep the evidential status clear. Patient-reported symptoms remain subjective even when organised under a history heading. Verified measurements remain objective. The crucial rule is that interpretation should not be hidden inside either evidence section.

How concise should a SOAP note be?

Concise means limited to information that supports the clinical problems, assessment and plan. It does not mean deleting uncertainty, monitoring or the reason for action. Another clinician should be able to continue care without reconstructing the encounter from assumptions.

What makes a plan auditable?

An auditable plan names the action, intended outcome, monitoring measure, review interval, responsible person and earlier escalation trigger. Generic phrases such as counselled or follow up record activity but not the change that should result.

Study strategy

Assessment move

Write a note from an invented five-minute encounter. Highlight S and O in different colours, underline every interpretive phrase in A, and draw a line from each P item back to its supporting assessment and evidence. Any assessment sentence with no evidence needs support or removal. Any plan item with no assessment is an orphaned action.

Add a monitoring measure, review interval, responsible person and early escalation trigger to every continuing problem. Then edit for concision without deleting the source status or uncertainty that makes the note defensible. Ask another reader to distinguish what the patient reported, what was measured and what the pharmacist concluded. If the categories blur, repair placement and wording before polishing grammar.

Finally compare the note with the original encounter to ensure compression did not change the patient's meaning or turn a possible explanation into a confirmed fact. Number the assessed problems temporarily and place the same number beside every linked plan item; missing or duplicated numbers reveal broken traces before prose editing begins.

Write one assessment that includes a likely explanation, an important alternative and the evidence limit separating them. Ensure that the wording does not imply certainty beyond the available history and observations. Give the note to a reader who did not hear the encounter and ask what they would do first, what they would monitor and when they would escalate.

A different answer from your intention signals a documentation defect, not reader failure. Reorder the plan into immediate action, patient instruction, follow-up and contingency where that improves usability. Compare the final S section with the patient's actual phrasing so compression does not erase the main concern. For O, record only observed, measured or verified material and retain relevant method context.

Finish with a deletion pass in which every removed sentence must be redundant rather than merely inconvenient; concision is successful only when the clinical route remains recoverable.
