Non-Dietary Factors and Reading Pooled Evidence

Habits beyond the plate

Week 5 turns to non-dietary factors before and during pregnancy: physical activity, sleep and mental health for both parents, with smoking and blood pressure on the maternal side, and their links to pregnancy outcomes and to children's neurocognitive, cardiometabolic and growth outcomes.

The maternal lecture uses each factor to teach one tool for reading evidence.

Measuring activity

Large studies express activity in metabolic equivalents.

One MET is resting energy use; moderate intensity is 3 to 5.9 METs and vigorous is 6.0 or more; multiplying intensity by minutes gives MET-minutes, so 3 METs for 30 minutes is 90. For maternal activity and offspring neurodevelopment, the lecture's example review found no effect in randomised trials and a positive association in the cohort studies, which is used to introduce the hierarchy of evidence with systematic reviews and meta-analyses at its apex.

Pooling studies

Maternal smoking and low birth weight is the case for reading a meta-analysis.

The source review pooled 55 cohort studies spanning publications from 1986 through 2020. A funnel plot checks whether small studies scatter symmetrically around the pooled result; pronounced asymmetry suggests publication bias, and the smoking plot showed none of significance. I squared expresses how much of the variation between studies reflects real differences rather than chance.

A forest plot shows each study's estimate and interval beside the line of no effect and ends in the pooled estimate. The lecture concludes that smoking in pregnancy is directly associated with low birth weight.

Sleep, blood pressure and study quality

Reported sleep shortened across pregnancy in the lecture's example cohort, and poorer sleep was associated with more ADHD symptoms in children at four.

For hypertensive disorders of pregnancy and adult offspring outcomes, the lecture teaches the Newcastle-Ottawa scale, which awards up to five stars for selection, two for comparability and three for outcome.

The two readings

A 2026 Japanese cohort of 8,238 mother-child pairs found that moderate and low maternal cardiovascular health in pregnancy, scored on Life's Essential 8, were associated with more developmental delay at four than high cardiovascular health.

A 1997 American study of 854 young adults found that before age three a child's obesity predicted little and parental obesity predicted more, with the child's own weight gaining importance with age.

Classifying a week of activity by intensity

Q [5 marks]. A participant reports cycling at 7 METs for 40 minutes twice a week and walking at 3.5 METs for 20 minutes on four days. These are AskSia's own values. Compute her weekly MET-minutes and classify each activity. The step weighting shown here is an AskSia study allocation and is not a University marking scheme.
Figure 8.1.Figure 8.1. A conventional evidence pyramid. SR and MA, the systematic review and meta-analysis, sit at the apex; the width of each tier suggests how many such studies exist. SR and MA randomised trials cohort studies case-control, cross-sectional stronger evidence at the top more studies

  • 2Compute the cycling total.
  • 2Compute the walking total.
  • 1Add them and classify each activity.
Cycling gives 7 × 40 = 280 MET-minutes a session, and 280 × 2 = 560 a week. Walking gives 3.5 × 20 = 70 MET-minutes a day, and 70 × 4 = 280 a week. The weekly total is 560 + 280 = 840 MET-minutes. Cycling at 7 METs is vigorous, because it is 6.0 or more; walking at 3.5 METs is moderate, within 3 to 5.9.
Sia tip — Multiply in a fixed order, METs, then minutes, then sessions, and write each product before adding. Arithmetic slips cluster at the addition step.
Glossary

Key terms

Meta-Analysis
A statistical combination of results from several studies into one pooled estimate.
Publication Bias
Distortion arising when studies with unfavourable or null results are less likely to be published.
I Squared
The share of variation across studies in a meta-analysis attributed to real differences rather than chance.
Forest Plot
A chart of each study's estimate and interval, with a pooled estimate at the foot.
Newcastle-Ottawa Scale
A star-based tool for rating the quality of cohort and case-control studies.
PRISMA Diagram
A flow chart showing how records were identified, screened and included in a systematic review.
FAQ

Non-Dietary Factors and Reading Pooled Evidence FAQ

What does a funnel plot show?

Each study's estimate against its size or precision. Large studies cluster near the pooled value and small ones scatter more widely beneath them; an asymmetric shape, with small studies missing on one side, suggests publication bias.

Why can trials and cohorts disagree about activity in pregnancy?

They protect against different problems. Trials assign activity, which removes confounding, but may be short; cohorts observe long habits in real life but active mothers may differ in other ways. The lecture's example review found no effect in trials and a positive association in cohorts.

What does the Newcastle-Ottawa scale score?

Three areas of a cohort study: selection, worth up to five stars, comparability, up to two, and outcome, up to three. One star is awarded for each criterion met, giving a quick, comparable judgement of study quality.

What did the Japanese reading on maternal cardiovascular health find?

Compared with high cardiovascular health in pregnancy, moderate and low cardiovascular health were associated with higher risk of developmental delay at four, with risk ratios of 1.30 and 1.62. Delay was reported by mothers on a screening questionnaire, which the authors list as a limitation.

Why does parental obesity matter more for toddlers?

In the 1997 reading, a one- or two-year-old's own obesity did not significantly predict young-adult obesity, while having an obese parent did. As children grew, their own obesity became the stronger predictor and the parents' status mattered less.

Study strategy

Assessment move

Learn the three plots as a sequence of questions: is anything missing, do the studies agree, what is the combined answer. Then practise MET arithmetic with your own week until the multiplication order is automatic.
