Developmental Origins of Health and Disease

A claim about sensitive periods

The course opens with a single idea and spends thirteen weeks testing it. Early development can set a person's later risk of disease. Stated about fetal life, this was the Fetal Origins of Adult Disease hypothesis; broadened to early childhood and to health as well as disease, it became DOHaD.

The Singapore description quoted in the lecture names environmental, nutritional, hormonal and metabolic factors acting in sensitive windows, and says their effect on the growing fetus and child may be lasting.

Where the idea came from

The origin reading is a 1986 comparison of 212 local authority areas in England and Wales.

Areas with high infant mortality in the early 1920s had high heart disease mortality in the late 1960s and 1970s, a positive correlation seen in both sexes, across ages and across regions. The lecture adds the detail that decides how to read it: the infants who died are not the adults who died later. The study compared places, so it generated the hypothesis without testing it in any individual.

Its authors also allowed that some third factor might link the two rates.

Why timing matters from the start

The lecture divides pregnancy into three windows. In the first trimester all major organs begin to develop and the embryo is most vulnerable to deficiencies and harmful exposures; the second trimester is growth and specialisation; the third is maturation.

That division is the reason later chapters keep asking when an exposure happened, not only whether it happened.

Three extensions

The father enters through Paternal Origins of Health and Disease, which holds that a man's exposures before conception can modify the sperm epigenome and affect his children.

Mental health enters through Developmental Origins of Mental Health and Disorders, which traces major psychiatric disorders to development. The curriculum also extends the idea to how long people live, and how long they stay healthy.

The lecture ends by noting that the hypothesis met strong scepticism until epigenetics offered a mechanism, and that new birth cohorts and research in lower-income countries remain the main gaps.

Separating an ecological association from an individual claim

Q [5 marks]. A report says the 1986 study proved that babies who were poorly fed became adults with heart disease. Identify the study's unit of analysis and rewrite the claim so it matches the design. The step weighting shown here is an AskSia study allocation and is not a University marking scheme.

  • 1Identify the unit of analysis.
  • 2Explain why the infants and adults are different people.
  • 2Rewrite the claim at the level the design supports.
The unit was the area: 212 local authority areas, each with an infant mortality rate for the early 1920s and a heart disease death rate decades later. The infants who died were not the adults who later died, so no individual was followed from birth. A correct claim is that areas with poor conditions for infants later had higher heart disease mortality, an ecological association that suggests early-life nutrition may raise adult susceptibility and that individual-level studies then needed to test.
Sia tip — Ask whether the same person appears at both ends of the association. If not, write about places or groups, never about individuals.
Glossary

Key terms

FOaD
Fetal Origins of Adult Disease: the original claim that events during fetal development shape the risk of adult disease.
Sensitive Period
A stretch of development when a tissue is being built and can be permanently pushed by an exposure.
Ecological Study
A study whose unit is a group or area rather than a person, so its associations describe places.
DOMHaD
Developmental Origins of Mental Health and Disorders: the extension tracing psychiatric disorders to development.
Organogenesis
The formation of the major organ systems, which begins in the first trimester.
FAQ

Developmental Origins of Health and Disease FAQ

Why is the 1986 study called ecological?

Its data were rates for 212 local authority areas, not measurements on people. It linked the infant mortality of each area in 1921 to 1925 with that area's heart disease mortality in 1968 to 1978, so it can describe places but cannot say that any individual infant became a sick adult.

What is the difference between FOaD and DOHaD?

FOaD concerns fetal life and adult disease. DOHaD widens the window to early childhood, names a wider set of environmental, nutritional, hormonal and metabolic influences, and speaks of health as well as disease, which is why it became the umbrella term used across the course.

Why does the first trimester get special attention?

All major organs begin to form in weeks 0 to 12, and the lecture describes the embryo as particularly vulnerable then to nutritional deficiencies and harmful exposures. An exposure in that window acts on formation, whereas a late exposure acts mainly on growth and maturation.

What did epigenetics change for the hypothesis?

It supplied a possible mechanism. The lecture notes that the hypothesis was widely doubted until the field moved toward epigenetics, because an explanation of how an early exposure could persist made the long-delayed associations biologically plausible.

What research gaps does the first lecture name?

New or extended birth cohorts to check findings from animal models in people, and more work in poorer countries where nutrition problems are common. Intergenerational cohorts are presented as able to answer many of the open questions.
