Epigenetic Mechanisms and Early-Life Exposures

One genotype, several phenotypes

Week 1 defines epigenetics as the way one genotype can give rise to different phenotypes, a term coined by Conrad Waddington in 1942 and offered as a possible mechanism for developmental programming. Three processes are named: DNA methylation, histone modification and microRNAs, which are short non-coding RNAs of about 22 nucleotides.

Between early life and adulthood the lecture places an interaction of three things, the environment in the womb, the child's genetic makeup and the epigenetic layer.

Identical twins matter because they hold genes constant, which makes them a more rigorous test of environmental effects than general population studies.

Animals can show cause, humans show relevance

Animal models allow the fetal environment to be changed deliberately, tissue-specific effects to be measured and other exposures to be controlled tightly, which is why they are critical for showing a causal link between fetal environment and epigenetic change.

The lecture warns that their findings do not always apply to humans.

The Agouti mouse is the standard example: methyl-donating nutrients in the mother's diet, such as folic acid, vitamin B12, betaine and choline, increase methylation and silence the Agouti gene, shifting offspring from a yellow, obesity-prone coat toward a brown, leaner one.

Diethylstilbestrol in the daughters

From 1938 until 1971, doctors prescribed diethylstilbestrol, DES, to millions of pregnant women.

In 1971 young women exposed before birth were found with a rare vaginal cancer usually seen in older women, and DES became the first cancer-causing agent shown to pass from mother to fetus and produce cancer in the child.

The 2011 reading combined three cohorts of 4,653 exposed and 1,927 unexposed women and compared twelve adverse outcomes, with every hazard ratio above 1, from preeclampsia to neonatal death.

Folate from mother and father

A neural tube defect is a malformation from incomplete closure of the neural tube, which happens about 28 days after conception.

Women planning pregnancy are advised to take 400 to 800 µg of folic acid daily, and Singapore recommends at least 400 µg before conception; food fortification helps where supplements are hard to reach. The lecture also shows men with the lowest folate status having poorer semen parameters than men with the highest.

Reading a cumulative risk and its hazard ratio together

Q [6 marks]. Exposed women had preterm delivery in 53.3% of deliveries against 17.8% of unexposed women, hazard ratio 4.68, 95% confidence interval 3.74 to 5.86. Interpret both numbers and name one reason the result is treated as strong evidence. The step weighting shown here is an AskSia study allocation and is not a University marking scheme.

  • 2Interpret the two cumulative risks.
  • 2Interpret the hazard ratio and its interval.
  • 2Give one design reason for confidence.
Just over half of exposed women who delivered had a preterm delivery, against fewer than one in five unexposed women. At any given age the rate among exposed women was about 4.7 times that among unexposed women, and the interval lies well above 1, so chance is an unlikely explanation. The result carries weight because exposure was documented in medical records rather than recalled years later, and because risks were higher again in women whose tissue changes marked heavier and earlier exposure, a dose-response pattern.
Sia tip — Say the direction before the size: above 1 means more risk in the exposed group. Then say whether the interval crosses 1.
Glossary

Key terms

Histone Modification
Chemical change to the proteins DNA is wound around, altering how tightly it is packed and read.
MicroRNA
A short non-coding RNA of about 22 nucleotides that damps the message of a gene after it is read.
Agouti Mouse
A mouse model whose coat colour and obesity risk reveal the methylation state of one gene.
Methyl Donor
A nutrient such as folic acid, vitamin B12, betaine or choline that supplies groups for DNA methylation.
Diethylstilbestrol
A synthetic oestrogen given to pregnant women until 1971 that harmed the reproductive health of exposed daughters.
Neural Tube Defect
A congenital malformation from incomplete closure of the neural tube about 28 days after conception.
FAQ

Epigenetic Mechanisms and Early-Life Exposures FAQ

Why are identical twins useful for studying epigenetics?

They share the same genes, so differences between them in phenotype point to the environment. The lecture describes monozygotic twins as a more rigorous framework for studying environmental effects than general population studies, where genetic and environmental differences are mixed.

What does the Agouti mouse demonstrate?

That a mother's diet in pregnancy can change gene expression in her offspring without changing the gene. Methyl donors raise methylation, the Agouti gene is silenced, and the offspring are brown and leaner rather than yellow and prone to obesity, so a visible trait tracks a metabolic one.

Why was DES historically important?

It was the first cancer-causing agent shown to pass from mother to fetus and produce cancer in the child. Its later follow-up showed that exposure before birth also raised the risks of infertility, pregnancy loss, preterm delivery and other outcomes, a wide reach from one pregnancy exposure.

Why is folic acid advised before pregnancy rather than after?

The neural tube closes around 28 days after conception, usually before a pregnancy is confirmed. Supplementation that starts at the first antenatal visit often arrives after the window it is meant to protect, so the advice targets women planning pregnancy.

Can mouse results be applied directly to people?

Not always. Animal models are strong for showing that a fetal exposure causes epigenetic change, because the exposure can be controlled, but the lecture cautions that their findings may not apply to humans. Human cohorts and trials are needed to confirm them.

Study strategy

Assessment move

Practise the Agouti chain aloud until the direction is automatic: more methyl donors, more methylation, a silenced gene, a brown and leaner mouse. Then state the DES result with one hazard ratio and its interval.
